If you have out-of-network benefits, I can provide documentation for you to submit to your insurance company for possible reimbursement.

Please note that reimbursement is determined by your insurance plan, not by my practice.

Good Faith Estimate

Under federal law, people who are uninsured or who do not intend to submit their care to insurance may be entitled to receive a Good Faith Estimate of expected charges.
